We will discuss the importance of obtaining an adequate history from parents/caregivers as a means to direct your physical examination and inform your differential diagnosis. Additionally, we will discuss physical examination tactics/techniques to make the correct diagnosis and guide your plan of care.
Toe Walking – Idiopathic vs. Pathological

Toe walking is a common problem presenting to pediatric and orthopedic practices. This presentation will include a review of the differential diagnosis of toe walking. Specific attention will then be given to idiopathic toe walking, the most common type of toe walking. Attendees will learn recent updates on the natural history, genetics, and treatment of idiopathic toe walkers.
Learning Objectives:
- Define toe walking, explain nomenclature, and differential diagnosis.
- Describe the evaluation of a child with toe walking with respect to history and physical examination. Be able to identify “red flags” that would warrant further referral.
- Discuss management, both nonsurgical and surgical, of idiopathic toe walking.

Pes planovalgus is a common condition in children, and must not be treated as a small version of the adult flatfoot deformity. Most flexible flatfeet are physiologic, asymptomatic, and require no treatment, however, some may be rigid and may be associated with neuromuscular diseases, tarsal coalitions, and accessory navicular syndrome. The most important challenge for the physician is to distinguish a condition that may have a benign natural history from those that may cause disability if left untreated. Successful management of pediatric flat foot deformity can be nonsurgical or surgical.
Sports Injuries, Stress Fractures and Female Triad

Clubfoot – Ponseti Technique and Beyond

There are four elements of clubfeet: equinus positioning of the hindfoot, varus positioning of the calcaneus, a cavus arch, and metatarsus adductus. The common goal of treatment throughout the ages has been to correct these deformities and obtain a foot that is normal in appearance, function, mobility, and pain free. Manipulation, surgical interventions and combination of manipulation and surgical intervention have been described to attain this. It is the goal of this lecture is to briefly review the history of the treatments for clubfoot, discuss its pathogenesis and classification systems, describe treatments, mainly Ponseti technique, that has provided the most successful outcomes, and review complications of treatment and relapse and ways to avoid them.
Surgical and Conservative Management of Tarsal Coalitions

Tarsal coalitions are recognized as a congenital anomaly whereby two or more bones of the hindfoot and midfoot are fused resulting in limitation of foot motion and pain. Tarsal coalitions were found to be the cause of painful flatfeet in adolescents and young adults, as well as the possible cause of routine ankle sprains due to the limitation of range of motion. The most commonly noted tarsal coalitions in the foot and ankle are calcaneonavicular and talocalcaneal coalitions. In this lecture, we will be discussing a step-wise conservative and surgical appropriate to the management of tarsal coalitions that have been shown to provide excellent long-term benefits.
