The longitudinal arch of the human foot is a complex mechanical structure that must be compliant on uneven surfaces and also have sufficient stiffness to allow the foot to be an efficient propulsive organ during walking and running gait. To serve these functions, the longitudinal arch has a unique four-layer load-sharing system consisting of the plantar fascia, plantar intrinsic muscles, plantar arch extrinsic muscles, and plantar ligaments. These four layers of tension load-bearing elements, working together with the osseous elements which serve as the framework of the longitudinal arch, work synergistically to increase longitudinal arch stiffness during weightbearing activities.

Prior to the Covid epidemic, Podiatrists were becoming more involved in Sclerotherapy for the treatment of superficial venous disease. Based on past history it’s the speaker’s assumption that this will increase as Covid becomes more resolved.

Following this introduction, the session covers the

  1. basic principles of the application of Sclerotherapy.
  2. A comprehensive overview of articles and the overwhelmingly positive results reflected in these papers.
  3. It follows with a case review dealing with a major complication (sclerotherapy) nearly resulting in a BK amputation.
  4. The session poses the question as to who should be involved in Sclerotherapy treatment, being prepared for complications, and how in this case significant factors were overlooked which nearly led to the loss of a leg. It’s a red flag – a caution – Be very careful when getting involved in new therapies until you understand all potential ramifications.
